New Grant Application Form

If your organization has been funded in the past by the Foundation, please fill out the form below. If your organization has NOT been funded in the past by the Foundation, please fill out the Prequalification for New Grants form instead.

Required Submissions
In addition to filling out the online form below, two copies of the following items must be mailed to us at P.O. Box 16909, Golden CO 80402. Please note that incomplete proposals will not be reviewed.
  1. An introductory letter describing the purposes and amount of your request.
  2. A narrative (no more than three pages) describing when your organization was founded, its history, what it is trying to accomplish, and what organizations it partners with to do its work.
  3. Your organization's 501(c)(3) designation from the Internal Revenue Service.
  4. Your organization's most recent audited financial statements; if not available, a copy of your most recent IRS Form 990 or 990EZ.
  5. An operating budget and a project budget (if applicable)
  6. A list of other funding sources for this request. Please list whether or not funding is committed, pending, or received from the additional sources.
  7. A list of the organization's major funding sources and the dollar amounts received for the past two years.
  8. A list of your organization's board of directors and their community and professional affiliations.
  9. An evaluation plan. What outcomes do you hope to achieve with the help of this grant? How will you measure them?
  10. Any recent printed materials including an annual report, media clippings, etc. (no more than five pages)
  11. How did you hear about the McDonnell Family Foundation?

Grant Limitations
  1. The Foundation makes grants to 501(c)(3) tax-exempt organizations eligible for funding under the United States Internal Revenue Code.
  2. To conserve limited resources, the Foundation does not consider more than one proposal from the same organization within a 12-month period.
  3. The McDonnell Family Foundation does NOT fund tickets for events, municipalities, or individuals.
  4. The Foundation only accepts grant requests from the states of California, Colorado, Kentucky, and Ohio.
